Emotional Support Animal (ESA)

CSU Policy on Service and Emotional Support Animals for Students on Campus outlines requirements for documentation that supports a student's request for an emotional support animal (ESA) in Cal Poly Humboldt Housing.  Documentation must be from a licensed medical or mental health professional dated within the last 30 days and must include:

  1. The provider's professional opinion that a condition qualifies as a disability (i.e., a physical or mental impairment which limits a major life activity);
  2. The provider's opinion that the emotional support animal has been prescribed for treatment purposes and is necessary to mitigate symptoms associated with the condition in order to have a reasonable accommodation to use and enjoy housing services;
  3. Any other information that would provide CDRC the ability to determine the appropriateness and need for the emotional support animal.  CDRC may require additional information or documentation.

NOTE:  Numerous internet services that suggest that they can provide necessary documentation for a fee can be be very misleading.  Before paying for any service that claims they may provide documentation necessary to support your need for an emotional support animal, please contact the CDRC office for guidance or reach out to your current qualified treatment provider for consultation.
